There are three different types of questionnaires to complete on this website:
- Strengths & Difficulties Questionnaire (SDQ)
- Therapy Session Rating Scale (TSRS)
- The Personal Wellbeing Index (PWI)
Welcome to this website for Core Psychology Services
We are collecting the outcome measures for the therapy teams in your school. If your child goes to one of the schools or you have been given a link, then please complete the SDQ (Strengths & Difficulties Questionnaire), TSRS (Therapy Session Rating Scale) and the PWI (Personal Wellbeing Scale).
The questionnaires help the therapy team know if they are making an impact within school and home life for your child. The questionnaires are looking at symptom reduction, overall happiness and how they feel about therapy.

The questionnaires are quick and simple to complete. Some of the questionnaires are for:
- Parents
- Teachers
- Children
Once you have completed the questionnaire and pressed 'Submit' then you are finished with that questionnaire.
